Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP)

Who determines what visit types to include in this report? We typically address BMI at the yearly wellness visit. Should we continue to use only the yearly wellness visit or should we include ALL office visits (sicks visits & immunization visits)?
