      Thank you for your inquiry regarding CMS56v12: Functional Status Assessment for Total Hip Replacement. For the numerator criteria, the measure only requires documentations of assessments performed; the assessments are not tied to a specific encounter and are not limited to a specific provider type. To meet the initial population/denominator and numerator, the measure looks for documentations where patient had a primary THA and an outpatient encounter within the specified time frames, and an initial assessment and a follow-up assessment before and after primary THA within the specified timeframes.

    • We thought we could report this measure in 2024 but now think we can't

      We want to be sure that the reporting provider must see the patient for each of the two office visits this measure mentions where the assessment tool must be used.  Our issue is that the reporting provider sees the patients for the pre-operative visit and has the first assessment done at that time, but does not do the post-surgical encounters; the physician assistant is tasked with those visits. If the reporting provider himself doesn't see the patient for the visit 300-425 days post-surgery for the second assessment, I want to confirm that he cannot report on this measure.
