      Thank you for your question regarding CMS68v13, Documentation of Current Medications in the Medical Record. Immediate resources will vary depending on the clinician and health system. In your example if a patient did not provide medication information but a medication list is available through a connected pharmacy or primary care practice within the health system, the clinician should pull the medication list from those available resources to conduct the medication review. If those resources are not immediately available, the clinician is not required to conduct additional outreach to obtain a medication list.

      I am wondering in the measure it states using all immediate resources available to document medications for patients.  If patient doesn't bring in dosage, frequency, route info are we required to track down this info from pharmacy or primary doc?  Or will we meet measure with the data the patient provides?
