If those resources are not immediately available, the clinician is not required to conduct additional outreach to obtain a medication list.

I am wondering in the measure it states using all immediate resources available to document medications for patients.  If patient doesn't bring in dosage, frequency, route info are we required to track down this info from pharmacy or primary doc?  Or will we meet measure with the data the patient provides?
