      Thank you for your question on CMS177v12: Child and Adolescent Major Depressive Disorder (MDD): Suicide Risk Assessment. The measure specification does not require a specific assessment tool or instrument. The specific type and magnitude of the suicide risk assessment is intended to be at the discretion of the individual clinician and should be specific to the needs of the patient. At a minimum, suicide risk assessment should evaluate:
      1. Risk (e.g., age, sex, stressors, comorbid conditions, hopelessness, impulsivity) and protective factors (e.g., religious belief, concern not to hurt family) that may influence the desire to attempt suicide.
      2. Current severity of suicidality.
      3. Most severe point of suicidality in episode and lifetime.

      I'm not sure if this is the correct place for this inquiry. I work for the University of Virginia and we are applying for PCMH reaccreditation at one of our clinics. Our providers had a question about CMS-177 and we were told to reach out to Mathematica, who told us to reach out here. Our providers would like some feedback about the following: For Behavioral Health: Child and Adolescent Major Depressive Disorder (MDD): Suicide Risk Assessment (standardized measure): %patient visits for those 6-17 years with a diagnosis of major depressive disorder (MDD) with an assessment for suicide risk.

       

      Can you give us an example of a validated suicide screening tool for kids as young as 6 (which is the age they propose starting suicide risk assessment in the standardized measure)? Please let me know if you have any information or direction you can provide me, thank you!
