      Thank you for your question. For CMS2v12, Preventive Care and Screening: Screening for Depression and Follow-Up Plan, the measure looks for the most recent screening. In your scenario, the patient has been diagnosed with depression on their first screening. If the patient was screened a second time during the measurement period, the patient would meet the denominator exclusion (i.e., patients who have ever been diagnosed with depression or with bipolar disorder at any time prior to the qualifying encounter). There are scenarios in CMS2v12 (PY2023) where the most recent screening would count towards the numerator. This would occur if the patient's first depression screening was negative. If the patient was screened again during the measurement, the second (i.e., most recent) screening would count towards the numerator.
      For CMS2v13 (PY2024), "patients who have ever been diagnosed with depression" has been removed from the denominator exclusion. In your scenario, the second depression screening would count towards the numerator.

      For depression screening and follow-up, it states to take the most recent depression screening during the measurement period. But it also states that "The intent of the measure is to screen for new cases of depression in patients who have never had a diagnosis of depression or bipolar disorder prior to the qualifying encounter used to evaluate the numerator." So if a patient had a screening during the measurement period (screening 1) and was diagnosed with depression on the encounter, then had another screening (screening 2) during the measurement period, what screening should be reported on?
