      Thank you for inquiring about ePC-07, Severe Obstetric Complications. The list of severe obstetric complications is associated with two value sets - Severe Maternal Morbidity Diagnoses (2.16.840.1.113762.1.4.1029.255) and Severe Maternal Morbidity Procedures (2.16.840.1.113762.1.4.1029.256). Within each value set are several diagnoses and procedures that further specify the severe obstetric complications along with their associated SNOMED and ICD-10 codes. These can be found on the Value Set Authority Center (VSAC) website here: http://vsac.nlm.nih.gov. Search for them by their numerical OIDs listed in the Terminology section within the measure specifications, and they can also be downloaded to an excel spreadsheet from the VSAC.
       
      Each diagnosis and procedure listed is defined by a group of discrete terms, each term associated with an ICD-10 or SNOMED code. The presence of any one or more coded terms qualifies as a Severe Obstetric Condition. For Ventilation, search for Severe Maternal Morbidity Procedures (2.16.840.1.113762.1.4.1029.256) in the VSAC website. Select the Grouping Members button to see the list of 8 value sets; two are for Ventilation: one with three (3) ICD-10 codes and the other with three (3) SNOMED codes.

      For Disseminated Intravascular Coagulation, search for Severe Maternal Morbidity Diagnoses (2.16.840.1.113762.1.4.1029.255) in the VSAC; select the hyperlinked OID; then click on the Grouping Members button; and the Disseminated Intravascular Coagulation will be listed among the 17 value set grouping members. When its hyperlinked OID is selected, 28 ICD-10 codes will display.

      Please note that you will need login credentials to access the VSAC, and an account may be obtained through the UMLS Terminology Services (UTS). You can sign up via this site: https://uts.nlm.nih.gov/uts/signup-login.     • Some of the severe obstetric complications are concerning and the description field explains why.

      I'm writing regarding the severe obstetric complications (PC07) measure.  When I go to the CMS website noted above, it doesn't define the severe obstetric complications. For example, a patient who has an emergency C-section may have to be intubated for a short time for the c-section. Can you please clarify how ventilation is defined? Would you want this counted as a severe obstetric complication? We have noticed that Gestational Thrombocytopenia will come up as DIC as a SOC. However, GTP is a known and unpreventable complication of pregnancy, that does not often need to be treated and is not clinically a way that we diagnose DIC. Can you please clarify how Disseminated Intravascular Coagulation is defined? Also, do you have an audit tool that we can use to review these patients? Thank you.
