Thank you for your inquiry on CMS2v13: Preventative Care and Screening: Screening for Depression and Follow-Up Plan. We do not provide interpretation of negative or positive screening results due to the number of different screening tools. This measure is not prescriptive as to which depression screening tool is used and the measure guidance provides a list of examples of standardized depression screening tools, including the Patient Health Questionnaire (PHQ-9). Please refer to the screening tool itself or contact the tooling author for clarification/interpretation of the results. We also recommend that you document in your EHR if the patient depression screening is negative or positive in case of a CMS audit.

CMS-2 Depression Screening and Follow Up: The specification guidance states that depression screening does not require the documentation of a specific score, just whether the results are considered ‘positive’ or ‘negative’. If organizations are using a validated tool such as a PHQ-9 and the organization has outlined what constitutes a positive screening (score > or equal to 9) vs. a negative screening (score <9), can organizations go off of just the score itself to determine positive/negative screening? Or does the organization have to specifically identify a positive or negative screening for each individual patient?
