      STK-2 use the "Medication, Discharge" datatype, which for this measure contains two (2) value sets “Antithrombotic Therapy for Ischemic Stroke (OID 2.16.840.1.113762.1.4.1110.62) and "Pharmacological Contraindications For Antithrombotic Therapy (OID 2.16.840.1.113762.1.4.1110.52)" both refer to the discharge medication list and is intended to express medications ordered for post-discharge use and as well as assessing for the authorDateTime of medications discharged per episode during the measurement period. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/ . Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set.

      I was reviewing our eCQM for STK-2 and 5 prior to submission. In STK-2, I clearly saw that an antithrombotic was ordered on the day of discharge but yet the case failed the measure. When I asked our IT, I was told that the order was reconciled after the patient was discharged.
      What are the data elements required for this measure besides the antithrombotics being ordered on discharge? Do we need to have the MDs reconcile those medication within a certain time period?
