I was reviewing our eCQM for STK-2 and 5 prior to submission. In STK-2, I clearly saw that an antithrombotic was ordered on the day of discharge but yet the case failed the measure. When I asked our IT, I was told that the order was reconciled after the patient was discharged. What are the data elements required for this measure besides the antithrombotics being ordered on discharge? Do we need to have the MDs reconcile those medication within a certain time period?
