Frailty Diagnosis or Symptoms in Patient Active Problem List

      The advanced illness and frailty criteria for this measure is a denominator exclusion criterion. It is acceptable to count a frailty diagnosis or symptom that started prior to the measurement period and continued during the measurement period as meeting denominator exclusions criteria. Consider that ‘Frailty’ can be indicated by a device, a diagnosis, an encounter or a symptom.

      The measure logic looks for documentation of the following QDM Datatypes/codes where the prevalence periods overlap the measurement period. To explore the values that support the ‘Frailty Diagnoses’ you can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Select the “Search Value Sets” tab and enter the value set ID to review codes:

      "Frailty Device” value set (2.16.840.1.113883.3.464.1003.118.12.1300) or

      "Frailty Diagnosis" value set (2.16.840.1.113883.3.464.1003.113.12.1074) or

      "Frailty Symptom" value set (2.16.840.1.113883.3.464.1003.113.12.1075) or

      "Frailty Encounter" (2.16.840.1.113883.3.464.1003.101.12.1088)
