Palliative Care Consult Order vs. Comfort Care Orders

    • Patients with orders for palliative care consults only are being included in the denominator exclusions for 'comfort measures'

      We have been meeting with our clinical analytics team and would like some clarification on if having an order for a palliative care consult would be included as a comfort measures intervention. Someone from our clinical analytics team pulled up these notes for abstraction (below), and from that definition I believe that only having an order for a palliative care consult (this is an order to have the palliative care physicians / APNs meet with the patient and/or family to discuss plan of care and their preference on code status) would NOT be considered as a comfort measures intervention.

      Notes for Abstraction: Only accept terms identified in the list of inclusions. No other terminology will be accepted.
      Physician / APN / PA documentation of comfort measures only (hospice, comfort care, etc...) mentioned in the following contexts suffices:

      • Comfort measures only recommendation
      • Order for consultation or evaluation by a hospice care service
      • Patient or family request for comfort measures only
      • Plan for comfort measures only
      • Referral to hospice care service
