I am reviewing clinical measure requirements for CQMS ID 144 Oncology: Medical and Radiation - Plan of Care for Pain. The measure specifications indicate that a documented plan of care may include: use of opioids, nonopioid analgesics, psychological support, patient and/or family education, referral to a pain clinic, or reassessment of pain at an appropriate time interval. However, if the patient is already on a pain regimen or the pain is chronic and being managed by another specialist, is it clinically appropriate to document a plan of care for pain as "no action needed" if the patient is already having pain managed, but still continues with chronic, but manageable pain?
