      Thank you for your inquiry for CMS156v12:Use of High-Risk Medications in Older Adult. Similar to other eligible clinician (EC) eCQMs, this measure is intended to be outpatient. However, as per measure specification, a specific setting for where a medication is expected to be administered/consumed is not required for numerator evaluation. For example: “Medication, Order”: “Anticholinergics, first generation antihistamines” is not setting specific. This piece of logic is only looking for an order of anticholinergics medication. We will consider the feasibility of adding medication setting specificity/constraint in a future measure version.

      Setting is where the medication is expected to be consumed or administered. Examples:

      • Inpatient - in the hospital during the current inpatient stay
      • Outpatient - in an outpatient facility such as an ambulatory surgical center
      • Community - in the ambulatory setting (e.g., at home)

      In CMS156, when evaluating the medication order logic, example:
      Medication, Order": "Anticholinergics, first generation antihistamines
      is it the intent of the measure to only evaluate orders from a specific setting? Although the CQL logic does not include setting attribute? Or setting should not be evaluated in these cases? Please clarify., your guidance would be greatly appreciated.
