In many of our clinics, a patient is asked to fill out a depression screening when they check in, while they wait in the lobby before their visit begins. When they are roomed, the staff member enters the answers into the EMR in structured fields (flowsheet), before the provider joins. When the provider joins, the provider reviews the results and creates/documents a follow-up plan if needed.
For patients who would be excluded for medical reasons, such as severe dementia, the patient (or their caregiver) might have already completed the screening before entering the exam room. Due to clinic staff's scope of practice, the staff member would not be able to determine that the "completed" screening was invalid and they would enter the results into the chart as usual.
In these scenarios, can a provider indicate a medical exception in the medical record after the screening has been completed, to effectively discount the screening?
