We are working on ensuring we meet the measure for our patients who screen positive for depression to receive an appropriate follow up plan. Because the measure has changed to include patients who already have a diagnosis of depression, these patients likely already have treatment in place (a current medication or current therapy). The patient can have a positive screening, but it could be improved from their last screening showing their current treatment plan is working. If the provider feels they should continue their current treatment plan, could we have a follow up plan that included something similar to "continue current medication and/or therapy until next appointment in 3 months where this will be reassessed"? This would reflect an appropriate time interval for reassessment and a continued treatment plan.
