      Thank you for your inquiry about CMS56v12 (Functional Status Assessment for Total Hip Replacement). The measure evaluates whether patients complete a patient-reported functional status assessment (FSA) before and after a Total Hip Arthroplasty (THA). Appropriate functional status assessments that may be counted towards the measure include:

      Veterans RAND 12-item health survey [VR-12]
      Patient-Reported Outcomes Measurement Information System [PROMIS]-10-Global Health
      Hip Disability and Osteoarthritis Outcome Score [HOOS]
      Hip Disability and Osteoarthritis Outcome Score with Joint Replacement [HOOS JR]

      One of the types of FSA may be used to towards the numerator requirement if performed within the appropriate timeframe and with a result. Please note that the same FSA instrument must be used for both the initial and follow-up assessment. This quality measure requires a functional status assessment via QDM datatype "Assessment, Performed" using the appropriate codes found in the measure logic, and also requires a non-null test result using QDM attribute "Result".     • Our health system is implementing workflows in preparation of CMS's THA requirements for the near future.

      For CMS 56, Our system has been planning on implementing the PROMIS10 Assessment for our health system. We also understood we needed the HOOS or HOOSJr assessments for pre and post op. Upon closer look at the specification, we need clarification if only one of the following forms needs to be filled out in the proper timeframes.  Also, do we need the forms both pre and post op?
