Enoxaparin Needing SubQ Route added

      Thank you for your inquiry regarding CMS104 Antithrombotic Therapy at Discharge. This measure does not use the route of administration concept within the logic. The value set Antithrombotic Therapy for Ischemic Stroke uses RXNORM codes, this value set utilizes the semantic clinical drug (SCD) concept which are generalizable drug concepts and route is not specified.
       
      Here is the logic for STK-2 for Medication Discharge which shows the measure does not use the route of administration:
      • TJC."Ischemic Stroke Encounter" IschemicStrokeEncounter
      • with ["Medication, Discharge": "Antithrombotic Therapy for Ischemic Stroke"] DischargeAntithrombotic
      • such that DischargeAntithrombotic.authorDatetime during IschemicStrokeEncounter.relevantPeriod

      Regarding long-term antithrombotic therapy for stroke prevention (STK-2), either aspirin or clopidogrel are most often prescribed at discharge; however, SQ enoxaparin may be continued after discharge for some ischemic stroke patients, e.g., frail elderly, bedridden.

    • Not having SubQ as a route in VSAC for the enoxaparin medication, it is causing fallouts for the eSTK measures when it is ordered on depart.

      Hello,

      We recently had a eSTK-2 encounter fail even thought the patient received enoxaparin at depart. Our third party vendor, Oracle (formerly known as Cerner), stated it failed because the allowable value set in VSAC only says "injection" and does not have a "SubQ" route; injection is not a route. Can we please get the SubQ route added for this measure because that is the only way the medication is administered. Just accepting "injection" as the only value set will not suffice because there are other routes, in the medical field, in which "injection" could refer to.
