      Thank you for your inquiry about CMS130v12 Colorectal Cancer Screening. Additional documentation is required to determine whether an incomplete colonoscopy is eligible for use in the measure. A colonoscopy may count towards the numerator criteria if the procedure performed is documented using any codes from the "Colonoscopy (2.16.840.1.113883.3.464.1003.108.12.1020)" value set and was performed within the measurement time frame. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/ . Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set. A result is not required for colonoscopy to meet the numerator criteria. Any future updates to the measure's logic and coding will need to be captured and based on information documented in structured fields documented in an Electron Health Record (EHR).

      The specifications state a colonoscopy needs to be performed but does not specify if it needs to be completed. The logic does not allow for this nuance. The procedure, performed value set and eCQM logic for colonoscopies does not have anywhere to specify that the colonoscopy was not completed. The logic is looking for CPT codes in charges, but those CPT codes are the same ones that are dropped even if the procedure cannot be completed (those charges will have a modifier of 53-discontinued procedure).

      All organizations could be impacted by this with inflated numerators. We need clarification if the intent of this measure is to count performed colonoscopies or completed colonoscopies. If the intent is to have completed colonoscopies, we need to have the eCQM logic updated to account for this.
