    • It is impacting the determination of which patients qualify for the numerator of the measure.

      Is the intent of the measure for a patient to qualify for the numerator if medication reconciliation was completed on the same date as the encounter or must the reconciliation be completed prior to the patient discharge time. 

       

      For example we have a patient seen in the emergency room on 08/25.  Patient had a discharge time stamp of 08/25 2:40pm.  The documentation of the current medication has a time stamp of 08/25 3:00pm.  Should the patient be in numerator?
