      Hello - 

      About 10% of our STEMI's, enter the ED without a STEMI and then become one on the 2nd or 3rd EKG. The measure has no specs for these situations and currently the ED arrival time is not a valid time to "start the clock" on these patients. If using the ED arrival time, all of these subsqeuent EKG STEMI's will be fallouts. The positive EKG STEMI should be the "start the clock" time. How do we build the measure with this consideration?
