Thank you for your question regarding CMS69 v11: Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan. To be eligible for the measure’s denominator population, the patient must have a non-telehealth encounter during the measurement period with a code from the “Encounter to Evaluate BMI” valueset (2.16.840.1.113883.3.600.1.1751). Also, per the measure guidance the height and weight must be documented by an eligible clinician and cannot be self-reported. The follow-up must occur during the measurement year using codes included in the following valuesets “Follow Up for Below Normal BMI" (2.16.840.1.113883.3.600.1.1528) or "Follow Up for Above Normal BMI" (2.16.840.1.113883.3.600.1.1525). Follow-up plans that meet numerator criteria can occur during a telehealth visit.

      We received the following question:

For follow-up plans documented during the measurement period (not during the encounter to evaluate BMI), are there any encounter-type requirements? Confirming that telehealth encounters are ineligible for the encounter to evaluate BMI only, because height and weight measurements are clinical actions that must be completed by an eligible professional or their staff and cannot be conducted via telehealth.

On the CMS's website it says this measure is not eligible for telehealth- does that include follow-up plans as well?
