CMS 104- STK - Discharged on Antithrombotic Therapy

      Thank you for your question regarding CMS104 Discharged on Antithrombotic Therapy. This measure uses the value set Antithrombotic Therapy for Ischemic Stroke 2.16.840.1.113762.1.4.1110.62. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set. This grouping value set consists of RxNorm concepts that represent a dose formulation to meet intent and are designed to broadly to meet various clinical scenarios. This value set does include several variations of Aspirin 81 and your local medications codes should be mapped to one of the RXNORM codes within the value set.

      The medication value sets developed for the program contain RxNorm codes that are Semantic Clinical Drugs (SCD), generic, and prescribeable. Semantic Clinical Drug (SCD) codes and are generalizable drug concepts. Each RxNorm clinical drug name reflects the active ingredients, strengths, and dose form comprising that drug. Your local medication codes need to be mapped to the SCD RxNorm codes found in the value set in order to satisfy the electronic clinical quality measure. We are unable to provide specific guidance related to the mapping of codes. We recommend you consult with your EHR vendor and clinical partners. If mapping is conducted, you should maintain documentation in case of a CMS audit. If there are relevant updates to RXNORM we will consider adjustments to the value sets for the next Annual Update.

    • This is affecting our rates for CMS 104.

      A patient who had an ischemic stroke was discharged with ASPIRIN BUF(CaCarb-MgCarb-MgO) 81mg. Will this count as part of the antithrombotic therapy? Is there an Rx norm and value set for this medication? Kindly advise. Thank you.
