      Thank you for your inquiry on CMS2v13 Preventive Care and Screening: Screening for Depression and Follow-Up Plan. This is a patient-based measure. It evaluates the care of a patient and assigns the patient in one or more measure segments or populations. If four providers share an EHR system, the overall care of patient would be attributed to all 4 providers, not per individual episode with each provider.

      Can you please provide additional clarification on how the solution provided in https://oncprojectracking.healthit.gov/support/browse/CQM-6304, aligns with the measure intent of being a patient-based measure and with the guidance that the Depression screening is required once per measurement period, not at all encounters? Why wouldn't the measure be in the Numerator for all of the providers who had a qualifying encounter with the patient during the measurement period?
