      Thank you for your inquiry for CMS139v12. The measure logic for the numerator looks for whether an assessment was performed; the assessment does not need to be tied to a specific encounter. The assessment should be encoded with any of the following codes from the “Falls Screening” (2.16.840.1.113883.3.464.1003.118.12.1028) value set. Equivalent services can be mapped to the value set. If the mapping is done, documentation should be maintained in case of a CMS audit.

      - Falls in the past year, LOINC 52552-7

      - Standardized fall risk assessment was conducted [CMS Assessment], LOINC 57254-5

      - History of falling; immediate or within 3 months [Morse Fall Scale], LOINC 59454-9

      - Fall risk assessment, LOINC 73830-2

      For further clarification, a specific screening tool/method is not required for the measure. If the assessment performed assesses whether the patient has reported a fall or problem with gait/balance, and is documented using/mapped to one of the codes within the “Falls Screening” value set, that is all that will be needed for the numerator criteria. The date/time of the fall risk screening assessment should also be documented to meet screening requirements, i.e., at least once within the measurement period. We hope this helps.
