      Thank you for your inquiry regarding CMS2v12: Preventative Care and Screening: Screening for Depression and Follow-Up Plan.



      The measure assesses the most recent depression screening completed either during the qualifying encounter or within the 14 calendar days prior to that encounter. A patient can be screened twice on the same the day. That said, one screening had to be done before another screening. Please ensure that the screenings were documented appropriately. If the most recent screening was the one with the positive result, a follow-up plan must be documented on the day of or up to two days following the qualifying encounter in order for the numerator criteria to be met.

    • Measure logic currently favors positive over negative if on same date. Will need to change configuration if negative should be favored over positive.

      We have a real-world case where we received in a QRDA Cat 1 file for a patient both a negative and positive depression screen result with the same date and time.  No follow up for the positive screen result was documented.  Should the patient be in the numerator in this case?
