"Medication, Active": "Dementia Medications" clarification

      Thank you for your inquiry for CMS165v12 Controlling High Blood Pressure. The exclusion looks for patients with both frailty during the measurement year and advanced illness in the last two years, where advanced illness can be demonstrated by taking dementia medications, which is expressed in the logic using "Medication, Active". The specification does not differentiate situations; per measure logic and value set if there is documentation that the patient was actively taking dementia medications during the evaluated timeframe, the patient would meet criteria for the Denominator Exclusion.


      Per QDM 5.6 (https://ecqi.healthit.gov/now-available-quality-data-model-qdm-v5.6-publication-2023-reporting/performance-period ), "Medication, Active" looks for medication being taken by the patient during the; relevantDateTime and relevantPeriod look for the time the medication is active on the medication record if the medication was given or taken at a single point in time, or the interval from when the medication is first known to be used (generally the time of entry on the medication list) to when the medication is no longer active, it would meet the criteria for the ‘Denominator Exclusion’ You can reference the measure specifications, that can be found at the eCQI Resource Center at https://ecqi.healthit.gov/

      Based on the measure's guidance (CMS165), the intent of the 'AIFrailLTCF.Has Dementia Medications in Year Before or During Measurement Period' exclusion definition logic is to identify if the patient has been taking dementia medications during the measurement period or the year prior. 
      Is the expectation to evaluate the patient's medication list (with meds start/end times) to identify  'Medication, Active' during the interval?
      In a scenario where a patient only received "Dementia Medications" during a hospital stay the year before the measurement period, but never took the medication after that - the medication was never part of the patient's medication list.

      Would it be appropriate to state the patient did not have 'Dementia Medications' Active during the evaluated interval?
