Patients that are discharged to inpatient rehab facilities are not a denominator exclusion and are counted in the numerator population. Additionally, the language of “Discharge to Another Hospital” is represented by the value set “Discharge To Acute Care Facility" (2.16.840.1.113883.3.117.1.7.1.87). The codes in this value set represent an acute care or specialty hospital. ​
