This value set includes community hospitals, tertiary referral hospitals, and acute care hospitals This value set does not include other facilities such as SNF or rehab facility.

      When looking at Denominator Exclusions for STK measures, one of the exclusions is "discharged to another acute hospital." Our EHR system has the mapping for this measure named "Discharged to acute care facility" instead of hospital leading to some confusion as to what should and should not be included. Do ONLY acute hospitals count towards this exclusion, or would other facilities (such as SNF, rehab facility, etc) count as well?
