Brief Description of Measure:
CMS951: Kidney Health Evaluation
Percentage of patients aged 18-75 years with a diagnosis of diabetes who received a kidney health evaluation defined by an Estimated Glomerular Filtration Rate (eGFR) AND Urine Albumin-Creatinine Ratio (uACR) within the measurement period
Description of Issue:
Screening is a patient safety issue in seniors because medication safety is informed by eGFR or kidney function to guide avoidance or dose adjustment of medications that are excreted and/or metabolized by the kidneys. Clinical experts suggest expanding the age range for the initial population, from 18-75 to 18-85.
