Brief Description of Measure: CMS149: Dementia: Cognitive Assessment Percentage of patients, regardless of age, with a diagnosis of dementia for whom an assessment of cognition is performed and the results reviewed at least once within a 12-month period Description of Issue: In version 12 of the measure, if documentation of a dementia diagnosis is entered into the EHR on the same day as the encounter but after the encounter ends, the patient will not meet Initial Population criteria. The measure steward proposes to modify the Initial Population logic to identify the day of the start of the dementia diagnosis and the day of the qualifying encounters, rather than the exact time of the dementia diagnosis and qualifying encounter.
