      Thank you for your inquiry regarding CMS871v3 (Hospital Harm- Severe Hyperglycemia).

      For Issue #1: The calculation engine Cypress automatically performs unit conversions. Therefore, 250 g/dL is converted to 250000 mg/dL during the calculation. In the certification and Cypress Validation Utility + Calculation Check (CVU+) tests, if g/dL is reported, you will see a program specific error that states "Unit 'g/dL' for QDM:LaboratoryTestPerformed (Glucose lab test) does not match expected units (mg/dL). Units must match measure-defined units."

      For Issue #2: Please use the HQMF flow for CMS871v3, a screenshot of which is attached. (https://www.hl7.org/implement/standards/product_brief.cfm?product_id=97). You can have an episode that could qualify for both the Denominator Exclusion as well as the Numerator. Ratio measures follow two separate flows for each side of the ratio (Denominator : Numerator).

      IPOP -> DENOM -> DENEX-> Denom:MO
      IPOP -> NUMER -> NUMEX-> Numer:MO
      Since the Numerator population inherits directly from the initial Population (and not from Denominator) an episode can meet both the Denominator Exclusion and Numerator conditions. Specific to this measure, however, exclusion cases that are in the Numerator, are eventually removed in the Numerator: Measure Observation and are not counted in the top number of the ratio.

      Hi Team,

      We have encountered two issues while performing Cypress validation using Vendor Patients in Cypress 7.1.1. Please find the uploaded QRDA_Files.zip.

      Issue 1: See attached QRDA file "CMS871v3_DenomExcl_SC8_2022.xml"

      Our expectation is the Denominator Exclusion criteria must be false for this scenario. All Laboratory Tests are present in the QRDA file and three of them fail the criteria due to timing mismatch/incorrect result quantity(i.e. <1000), but there is one Laboratory Test that satisfies the timing and the result value but not the units of 'mg/dL' (provided units are 'g/dL'). This Lab of incorrect units is contributing to Exclusion.

      Issue 2: See attached QRDA file "CMS871v3_Num_SC41_2022.xml"

      Our expectation is the scenario must qualify only for Denominator Exclusion, but Cypress is also showing it is qualifying for numerator as per the attached screenshot. Kindly clarify if this is possible for an episode to qualify for both Denominator Exclusion as well as Numerator.

      Thanks,

      Auryl
