      Thank you for your inquiry regarding CMS143v11: Primary Open-Angle Glaucoma (POAG): Optic Nerve Evaluation.

       

      The measure intent is to promote examination and documentation of the structure and function of the optic nerve. To meet the numerator criteria, there needs to be a Cup to Disc Ratio Performed with Result documented, AND an Optic Disc Exam Performed with Results documented that identify optic dsic or retinal nerve abnormalities.

      The Cup to Disc Ratio (2.16.840.1.113883.3.526.3.1333) and Optic Disc Exam for Structural Abnormalities (2.16.840.1.113883.3.526.3.1334) value sets contain codes that correspond to examination of the cup to disc ratio and identification of optic disc or retinal nerve abnormalities.       I wanted to reach out to get clarification on Quality Measure 12/eCQM 143 Primary Open-Angle Glaucoma (POAG): Optic Nerve Evaluation for the PY 2023. The specification sheet states the patient needs to have an optic nerve head evaluation during one or more visits within 12 months. Is documentation of an optic nerve evaluation adequate for numerator credit or must there be specific findings documented?
