I have a client that uses a DT Screening for their all of their cancer patients. They would like to know if this is sufficient to use for the screening of depression and meet the measure requirements. They ask the following questions in the EHR - At risk for Depression? Y/N & At risk for Anxiety? Y/N). (please refer to screenshots attached.) 

In addition they also have the depression screening within their EMR. (please refer to screenshots attached.) 

Is the distress tool they currently use for COC and the questions for QOPI would be sufficient for screening? Where should they document the plan  if positive for depression?
