      Thank you for your inquiry for CMS155v12 Weight Assessment and Counseling for Nutrition and Physical Activity for Children/Adolescents. The intent of the measure is for a PCP or OB/GYN to perform the encounter/assessment in the Initial Population. The measure logic specifies, visits that would qualify for the Initial Population are those encoded with any codes listed in the value sets from the "Qualifying Encounters" definition. Patient reported height and weight metrics are allowed to be used in the quality measure providing the information is recorded in the EHR and is sufficiently accurate for use in clinical care. Determining the acceptability, reliability and validity of this patient reported information is left to the discretion of the clinician.

      For the CMS 155 quality measure - "Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents", can we add "patient reported" results for height and weight metrics in the EMR while collecting vital signs?
