      ​Thank you for your question on CMS104, Discharged on Antithrombotic Therapy. ​Only patients discharged to other acute inpatient facilities, as defined in the value set "Discharge To Acute Care Facility" (2.16.840.1.113883.3.117.1.7.1.87), are excluded. This value set includes community hospitals, tertiary referral hospitals, and acute care hospitals. Patients transferred to a rehab facility or other types of facilities are not excluded.

      One of the denominator exclusion for STROKE 2 is:

      Inpatient hospitalization for patients discharged to another hospital. 

       

      If our facility document that they discharge the pt to "Rehab" or to "other type of healthcare facility" will that be okay to exclude the pt., can we take them both as a exclusion criterion. If not, what are the documentation, you can accept for discharge to another hospital?
