      Thank you for your question regarding STK-2 CMS 104v11: Discharged on Antithrombotic Therapy. The IPP States "Inpatient hospitalizations for patients age 18 and older, discharged from inpatient care (non-elective admissions) with a principal diagnosis of ischemic or hemorrhagic stroke and a length of stay less than or equal to 120 days that ends during the measurement period."
      The Initial Patient Population is looking for non-elective inpatient encounters so if the patient was not a scheduled/elective encounter I would verify the mapping of direct admit and how its encoded in your ADT or admission source.

      We are missing a patient on our report who we think should be in the inpatient population. She is older than 18, discharged from inpatient care, with a principle diagnosis of I63.81 and a stay less than 120 days.  The only thing I can think of, that is keeping this patient out of the inpatient population, is that she was a direct admit from the provider's office, and she didn't go through the ER or come by ambulance from another facility.  Am I looking at this right, or could there be another reason this patient is not showing up on the report?
