      Thank you for your question regarding CMS122v12, Diabetes: Hemoglobin A1c (HbA1c) Poor Control (> 9%). The HbA1c test must be entered into an EHR and documented using a LOINC code found in the "HbA1c Laboratory Test" value set. This terminology requirement follows the CMS Measures Management System (MMS) Blueprint guidance to use LOINC code system for the clinical concept of "Laboratory Test".

    • EMRs that generate the Gcode on the claim but do not generate a corresponding loinc or snomed are missing performance.

      For the MIPS CQMs the numerators have GCodes. These aren't always a 1:1 to the eCQM valuesets, but when they are, is it appropriate to capture the numerator with the Gcode documented on the claim, as they aren't included in the valuesets for the eCQMs.
