      Thank you for your inquiry specific to CMS871v2 Hospital Harm – Severe Hyperglycemia eCQM. The intent of the measure is to only evaluate actual glucose levels to determine if it meets criteria. In addition to POC tests, glucose levels can be obtained by laboratory draw tests which would provide the actual glucose level. To meet the exclusion an initial result of >=1000 mg/dL must be captured in the specified timeframe, therefore if the initial glucose test result is >600 mg/dL by POC (or any other method), this would not meet the exclusion criteria. The result of a second serum test with a result >1000 mg/dL does not presume that the initial POC test of >600 mg/dL can be interpreted as also meeting the exclusion. We hope this helps.

      How do we account for hyperglycemia that is initially done by POC glucose?  The POC glucometer just goes to > 600 so it will not account for the >1000 until a serum is collected.  That means if serum comes back > 1000 it will be the second glucose.  Has this been discussed?  Is this something that can be discussed at annual if the measure needs to be reviewed for future?
