      Thank you for your inquiry regarding CMS68 v12: Documentation of Current Medications in the Medical Record. We can confirm that the measure does require an eligible clinician to attest to reviewing medications during each qualifying encounter, including if there are multiple encounters on the same day. For Scenario 1, we would expect the RN visit to fail the numerator criteria if the attestation by an eligible clinician is not performed during the visit. The second visit would also be included in the denominator and pass the numerator criteria. For Scenario 2, if all encounters are denominator eligible, we would expect the cardiology and rheumatology encounters to pass the numerator criteria and the hematology encounter to fail the numerator criteria.

      Hello,

      We are finding instances in which patients have more than one qualifying encounter on the same day.  Is the expectation attestation to documenting a list of current medications occurs at every eligible encounter if multiple are qualifying visits occur on the same day?

       

      Scenario 1:

       

      • Patient has an eligible nurse visit on 8/29/2023 with an RN to prep for subsequent visit with provider; no medication attestation occurs at this visit. Would this visit pass or fail numerator criteria for this measure?
      • Patient then seen by MD/PA/NP on 8/29/2023 for an eligible visit directly after the RN visit.  Medication attestation occurs at this visit.  Would this visit pass or fail numerator criteria for this measure?

       

       

      Scenario 2:

       

      • Patient has an eligible visit with cardiology on 8/29/2023 and medication attestation performed appropriately.  Would this visit pass or fail numerator criteria for this measure?
      • Patient has eligible visit with rheumatology on 8/29/2023 and medication attestation performed appropriately. Would this visit pass or fail numerator criteria for this measure?
      • Patient has an eligible visit with hematology on 8/29/2023 and medication attestation was NOT performed. Would this visit pass or fail numerator criteria for this measure?
