      In the December 2012 eSpec for VTE-3, there is a section that reads:
      AND:

      OR: "Laboratory Test, Result: INR (result >= 2 )" <= 1 day(s) starts before start of (MOST RECENT :"Medication, Administered: Parenteral Anticoagulant" during "Occurrence A of Encounter, Performed: Inpatient Encounter")

      In the equivalent section of the April 2013 version, it reads:
      AND:

      OR: "Laboratory Test, Result: INR (result >= 2 )" <= 1 day(s) starts before start of ("Medication, Administered: Parenteral Anticoagulant" during "Occurrence A of Encounter, Performed: Inpatient Encounter")

      The MOST RECENT qualifier has been removed. Was there a particular rationale for this? It seems to us that keeping the MOST RECENT qualifier in would be both a logistical and a clinical best practice (otherwise, the spec would seem to literally be referencing a therapeutic INR level prior to the start of Parenteral Anticoagulant).
