      1. For data entered in an EMR that is not tied to a code in the EMR, can an entity “map” that data to a code in a third part software? For example, if a hospital has an ED Admission Assessment with the decision to admit the patient in a form in their EMR, but does not have a code tied to that assessment in the backend, can they map that to an appropriate code in a third party software that aggregates and calculates the measures for them?
      2. For data entered in an EMR that is tied to a code, but the code system is not used in a measure, are facilities allowed to “crosswalk” that data. For example, if an EMR has a standard discharge status for a patient (such as 07: Left against medical advice or discontinued care) but the eCQM uses an OID that only contains a SNOMED (445060000: Left against medical advice (finding)), are we allowed to crosswalk those equivalent meanings? If we can crosswalk the codes, what do we need to be able to display for auditing purposes? The original code and the cross-walked code?
