Thank you for your question. The VTE Initial Patient Population includes patients with an ICD-10-CM Other Diagnosis Code for VTE, a Patient Age (Admission Date minus Birthdate) greater than or equal to 18 years, and a Length of Stay (Discharge Date minus Admission Date) less than or equal to 120 days. Patients with an ICD-10-CM Principal Diagnosis Code of VTE are not included in the Initial Patient Population. The ICD-10-CM Principal Diagnosis Code is defined as the diagnosis code that is primarily responsible for the admission of the patient to the hospital for care during this hospitalization.

The clinical intent of VTE-1 and VTE-2 is prevention of blood clots. Patients with a known blood clot at the time of admission to the hospital are excluded because therapeutic management rather than prophylaxis indicated. Patients with liver cancer or other cancers are not excluded from the VTE Initial Patient Population or VTE-1 and VTE-2 measures.

    • It affects my dissertation research because I am unsure about which measures to go forward with.

I am a Ph.D. student interested in doing research on cardiovascular eCQMs like VTE1/2. I want to understand the reason why the initial population does not include patient encounters with diagnoses of VTE.

I am working with the MIMIC III database. Here are scenarios I have questions on:

      • A patient came in with blood clots. They would need VTE prophylaxis. Why are they not allowed in the initial population?
      • A patient came in for liver cancer treatment as an in-patient. A blood clot was discovered the next day, and a venous thrombosis diagnosis was recorded.  It is not their present-on-admission diagnosis nor their principal diagnosis. Why is this not allowed in the initial population?

Thank you for any help or references you can provide. I am still new to this.
