      ​Thank you for your inquiry regarding CMS68 v12: Documentation of Current Medications in the Medical Record. Each visit with a qualifying encounter code during the measurement period will count in the measure’s denominator. The numerator action must be performed by a MIPS eligible clinician type ( https://qpp.cms.gov/mips/how-eligibility-is-determined#mips-eligible-clinician-types). If the numerator action is recording twice for a single visit, it could be related to how this measure is being implemented specifically in your EHR. The intent is for each denominator-eligible visit to be evaluated separately.

      Hello,

      We have seen a situation where patients have an encounter with primary care providers and on the same day, a Medicare annual wellness visit with an RN.  The documentation of current Medications is being counted twice for the visit with the primary care provider, and not for the visit with the RN Medicare wellness.  

      Are there any reasons to find an issue with this situation?  We want to ensure that there are no issues with coding, or that this will cause an error or problem in an audit.
