      Thank you for your question concerning CMS135v12 Heart Failure (HF) and CMS145v12 Coronary Artery Disease (CAD). If a discrete percentage for Ejection Fraction is not available in the medical record, the highest number in the range may be used. For these measures, an Ejection Fraction value of <=40% qualifies the patient for Moderate or Severe LVSD. Please let us know if you have additional questions.

      We have a question about the updated language that was added to the Guidance section of the 2024 CMS 135v12 and CMS 145v12 specification’s.  Specifically the second sentence about a text string not being recognized through electronic data capture. 

      If a report has a field for an Ejection Fraction on it and the result is defined as a range (e.g. 30-40%, Ejection Fraction 25-35%) and our logic can capture that range, is it acceptable to submit the upper value in the range on the QRDA file?

      From Guidance section:

      “In order for the Ejection Fraction result pathway to be recognized as below 40%, the result must be reported as a number with a unit of %.  A text string of “below 40%” or “ejection fraction between 35 and 40%” will not be recognized through electronic data capture.  Although, this criteria can also be met using the Diagnosis pathway is specified as “Moderate or Severe”.”
