Need to confirm if manual entry is eligible for eCQM repoting

If your question is in reference to manual entry of data elements such as lab values, and vital signs, then yes manual entry is accepted if the information is documented in a structured field in the EHR/EMR, and approved for entry by the provider. If your question is in reference to manual entry of something else for reporting on these measure we will need additional information.

    • There is no impact as such we just want to clarify if manual entries are allowed if the record exists in the EMR

      Need to confirm if manual entry is eligible for eCQM reporting for the following measures:

      ○ CMS122v10 Diabetes:
      Hemoglobin A1c (HbA1c)
      Poor Control (> 9%)
      ○ CMS2v11: Preventive Care
      and Screening: Screening for
      Depression & Follow-Up Plan
      ○ CMS165v10: Controlling High
      Blood Pressure
