The depression screening must be reviewed and addressed by the clinician, filing the code, on the date of the encounter. The date for a clinician reviewing the depression screening is not captured in the measure logic. We will consider adding this date in a future update.

      In ticket CQM-6141 (https://oncprojectracking.healthit.gov/support/browse/CQM-6141), you indicate that for CMS 2, while the patient can be screened for depression on the date of the encounter or up to 14 days prior to that encounter date, the depression screening must be reviewed and addressed by the clinician, filing the code, on the date of the encounter.

      My concern on this is how exactly is this represented in the QRDA Cat I. Looking at some files from Cypress Test Tool, I only see the date of assessment and then date of encounter, and as long as the date of assessment is within 14 days of encounter, it can count in the numerator. Attached file was run through the CVU+ tool and shows patient qualifies for numerator. Where do you show the date of the clinician documentation/finding?

      If it is required to review/document the code on the date of the encounter, I would think it would need to be represented in the QRDA Cat I. Otherwise, how can you enforce it or determine compliance? How would an EHR map this date of clinician review/approval into the QRDA Cat I separate from the date of the screening assessment?
