      My name is Brian Lopez and I serve as the Program Manager of Medical Informatics at Yuma Regional Medical Center. One of our providers has reached out in question of CMS 125. The provider wants to know if patient reported dates meet the requirement if the dates fall within the measurement period. I cannot find this information in the CMS website. If patient reported dates count, how do we map it appropriately in our EHR?
