      Hello. Thank you for reaching out with your question pertaining to CMS135, CMS144, and CMS145. The intent of the two qualifying encounters in the Initial Population of these measures is to capture patients who are being managed and followed by clinicians at a specific practice. In your example, patients may have encounter codes documented outside of your practice, but the intent is that they have two or more qualifying encounters performed by clinicians within your practice during the measurement period. Patients who are only seen one time during the measurement period for a consultation do not meet the qualifying encounter requirements for these measures.     • Adding SNOMED Codes

      All, For CMS 135 144 and 145, the value sets have SNOMED codes that represent encounters for an outpatient visit that is outside of our organization. Some of our providers only see a patient once a year to monitor their cardiac conditions (depending on their condition) while their primary care provider will see the patient at least annually as well. It is difficult to capture the Cardiology measures due to the fact the patient has to have 2 Qualifying Encounters. If our offices receive visit notes for an eligible encounter (comprehensive history, evaluation , and management of a patient presenting with minor to high severity problems) from the outside practices, records notes in the chart, is it appropriate to include as Qualifying Encounter for our provider's Quality measures for MIPS? Examples of the SNOMED codes are 30346009, 37894004 and 281036007 for CMS 135, 144, and 145.
