      Thank you for your question regarding CMS506v5 Safe Use of Opioids - Concurrent Prescribing. The intent of the measure is to capture opioids and benzodiazepines that are active medications on the patient's mediation list at the time of discharge. In this case the eCQM will capture the hydromorphone since it was an active medication the patient was taking at the time of discharge, even though there were instructions to stop taking it the next morning.

      Patient discharged to rehab. Discharge summary (date of service and discharge date/time 2/2, 2200) stated that hydromophone 2mg under ‘stop taking’ list. Physician added it to discharge summary as ‘stop’ taking on 2/3, 0900. Should eCQM report still capture the opioid since it was added as ‘stop’ on the next day after patient was discharged?
