To answer your first two questions, numerator 2 requires tobacco cessation interventions received during the measurement period or in the six months prior to the measurement period, therefore, the date/time of “Tobacco Cessation Pharmacotherapy Ordered” and “Active Pharmacotherapy for Tobacco Cessation” should be documented in order to meet the criteria. More details are provided below.

      The measure logic definition for “Tobacco Cessation Pharmacotherapy Ordered” requires a “Medication, Order” QDM datatype using a code from the value set “Tobacco Use Cessation Pharmacotherapy” where the “authorDatetime” (date/time the medication order is authored) is within the required timeframe.

      I have clarification on "Tobacco Cessation Pharmocology Ordered" and "Active Pharmacology for Tobacco Cessation" which are possible data elements to meet Numerator 2. Are there dates components for these two data elements? 

      For example, see this scenario: 

      • A patient is already taking an eligible medication for tobacco cessation (i.e., medication is in the active medication list without a date) due to patient history. A tobacco user screening resulted in positive. Would this meet the numerator requirement for Population 2?

      In addition, are over-the-counter nicotine therapy replacements are part of the tobacco cessation drug codes?
