      Thank you for your question regarding CMS69 v11: Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan. For your first question, a follow-up plan must be documented during the encounter or during the measurement period if the patient’s most recent BMI is outside of normal parameters.



      The case described in your second question would be compliant with the numerator as the last BMI measurement taken is outside of normal parameters and a follow-up plan has occurred and been documented during the measurement period.

      Two questions regarding this measure's PY6 specifications:
      1) The measure spec states that a follow-up plan must be "documented if MOST RECENT BMI was outside of normal parameters." Later in the spec, "when the BMI is outside of normal parameters, a follow-up plan is documented during the encounter or during the measurement period". Can you please clarify?
      2) The follow-up plan documentation for abnormal BMI no longer has to occur at the same encounter as the BMI measurement and can occur anytime during the measurement year. This means that the f/u documentation may occur before an abnormal BMI measurement when multiple BMIs are recorded throughout the year. Hypothetical example: Jan 2023 BMI 27, weight counseling and f/u plan documented; Oct 2023 BMI 24.5; Nov 23 BMI 26, no f/u plan. Per the measure spec, this case would be numerator compliant. Can you please confirm?
